The platform manages care provider registration, verification, matching, bookings, scheduling, care records and payments, allowing care providers to focus on what matters most providing safe, compassionate and reliable care to patients at home.
The Role
As a TrinityCare Home-Based Care Provider, you will provide appropriate healthcare and aftercare services to patients in their homes based on their individual care requirements.
You may support patients recovering from illness, surgery, hospitalisation or childbirth, as well as elderly patients, patients living with chronic conditions and individuals requiring ongoing assistance at home.
Your responsibilities may include:
Post-hospitalisation aftercare
Post-surgical recovery support
Personal care and daily living assistance
Medication reminders and medication support within your professional scope
Monitoring and recording vital signs where qualified
Wound care where appropriately qualified
Mobility and recovery support
Assistance with rehabilitation programmes
Maternal and postnatal support
Newborn support where appropriately qualified
Chronic condition support
Assistance with prescribed care plans
Patient observation and reporting
Accompanying patients to medical appointments where required
Supporting patients with activities of daily living
Providing companionship and emotional support
Communicating relevant observations to the patient's family or designated healthcare professional
Escalating changes in the patient's condition to the appropriate healthcare professional
Maintaining accurate care and visit records

Care Providers must only provide services that are consistent with their qualifications, training, professional registration, experience and approved scope of practice.

Qualifications and training
Professional registration, where applicable
Healthcare or caregiving experience
Patient groups they can support
Preferred service locations
All Care Providers are subject to TrinityCare's required verification and vetting process before being approved to accept patient bookings.
How TrinityCare Works
1. Register
Create your Care Provider profile and submit your qualifications, experience, references and areas of competence.
2. Verification
TrinityCare reviews your information and completes the required identity, qualification, reference and background verification.
3. Set Your Availability
Tell TrinityCare when and where you are available to provide home-based care.
4. Receive a Care Request
A patient, family member, hospital, clinic or authorised healthcare professional may request home-based aftercare through TrinityCare.
5. Get Matched
TrinityCare matches the patient with suitable care providers based on:

Review the care request and accept the assignment if it matches your skills, availability and approved scope of care.
7. Provide Home-Based Care
Visit the patient's home and provide the agreed care professionally, safely and respectfully.
8. Record the Visit
Complete the required care notes, observations, tasks completed and other relevant information through TrinityCare.
9. Escalate Concerns
If you observe a significant change in the patient's condition or identify a safety concern, promptly notify the appropriate family member and/or healthcare professional according to the care plan and TrinityCare procedures.
10. Get Paid
Payments are processed through TrinityCare in accordance with the applicable Care Provider agreement and payment terms.

Report changes in a patient's condition appropriately
Report incidents, accidents or safeguarding concerns
Never abuse, exploit or take advantage of a patient
Never perform services outside their approved scope of practice
